What exactly is accountability? In certain case, accountability means reporting to the certain authority about what was thought to be done, without the implementation. Sometimes, it seems like people are rather informing than to be accountable. After you doing something, you just merely inform the person about it. Shouldn't it be telling the person what to do before anything is being done?
